• Depending on the question, the number of selections you can make is specified (e.g., “Select only one option” or “Select all that apply”), so please answer within those limits.
• If you select “Others” or are answering an open-ended question, please provide specific details.
• You can save your answers temporarily so that you can resume where you left off later. Click the “Temporarily save” button at the bottom of the screen and follow the instructions that appear.
• If you close the screen while entering your answers, your answers will not be sent. Once you have finished answering the questions, be sure to click the “Register” button to proceed to the screen that says “This concludes the survey. Thank you for your cooperation.”
■ About Applying for Thank-You Gifts
As a thank-you gift for participating in the survey, we will hold a raffle to award 5,000-yen digital gift cards to 30 winners. If you would like to enter the draw, please enter your email address. We will notify the winners via email. We plan to notify winners in October.
